How do I fill out the location section of the service request form?
  1. In the address field, select the magnifying glass, and a map will open.
  2. Type in the NYC address, block, or intersection in the white field above the map. (Accepted location types vary by form. For specific options, please refer to the instructions provided within each form.)

If the location is an address, type in the building number, street name, and borough.

Example:

If the location is a block, type in the street/avenue the incident is taking place, type "between" and the cross streets of the incident, a comma and the borough.

Intersection

If the street is an intersection, type in the first street/avenue, type in "and", type in the second street/avenue.

  1. If the location typed is a valid location, the validated location will populate. Select the address. If the address is incorrect select cancel and type a new address.

If you're unable to pinpoint the exact location, you can use the map to drop a pin instead. Some forms also include an "Additional Location Details" field where you can provide extra context.
